GENERAL PURPOSE: Responsible for the oversight of Clinical Nurse Managers, Clinical Education, and clinical management/operations of Physician Practice Management primary and specialty practices of St. Lawrence Health System (SLHS). Works closely with the PPM Leadership Team Physicians and Advanced Practice Providers to create and support an environment where cutting-edge and innovative care is provided to a continually growing number of SLHS patients. MAJOR JOB OBJECTIVES AND RESPONSIBILITIES: % Time | Major Job Objective and description of responsibilities/milestones/key objectives | 35% | Management of Clinical Services | | - Assure implementation of patient care and quality assurance standards.
- Plan, organize and evaluate clinical practice and educational activities; development and implementation of policies, goals, and objectives and standards of nursing practice.
- Counsel and evaluate the performance of Clinical Nurse Managers and Nurse Educator. f. This includes staff development programs orientation to ensure compliance with practice standards.
- Identify and resolve clinical issues.
- Integrate goals, objectives, policy, process, and procedures with departmental clinical staff; advise and assist with the establishment and implementation of priorities.
- Work closely with medical staff and other disciplines for the successful implementation of the patient and institutional objectives.
- This includes an emphasis on Transitions of Care and Chronic Care Management to ensure the appropriate and seamless transition of patients from the inpatient to the outpatient setting while working collaboratively with hospital teams toward reducing readmissions.
- Provide guidance and assistance to Nurse Managers on counseling and discipline of clinical staff to enable achievement of department and organizational goals and objectives, particularly as related to high-quality patient care.
- Development and implement in-service guidelines for orientation and staff development. Encourage and provide opportunities for continuing education of the nursing staff.
- Participate in a concurrent and retrospective evaluation of patient care consistent with the organization's quality improvement program.
- Through this review process, evaluate and recommend changes and improvements in areas affecting the quality of patient care, in keeping with regulatory standards.
- Participate in interviews and recommendations for the selection of qualified personnel.
| 25% | Regulatory /Accreditation | | - Acts as PPM participant on assigned clinical SLHS and CPH Committees for DNV, Quality, Compliance, CMS, and other.
- Responsible for reporting PPM compliance with regulatory standards to appropriate SLHS departments.
- Develops action plans in collaboration with SLHS departments as needed.
- Maintain compliance with Federal and State regulations regarding affirmative action, safety, and patient care.
- Develops and maintains appropriate clinical standards policies and procedure manuals, protocols, guidelines, checklists, etc.
- Performs review and revision of P&Ps following review timeline standards or as changes occur.
- Acts as an expert resource in up-to-date clinical standards and ensures the rollout of changes and updates.
- Prepare sites for successful DNV Surveys.

- Oversees the development and implementation of staff training guidelines, seminars, and all other appropriate training.
- Participates in the development and implementation of department goals and objectives, which include the establishment of standards of practice consistent with national professional and regulatory guidelines.
- Initiate, plan, analyze, and participate in nursing staff development programs and workshops.
