JOB SUMMARY: The Supv Community Health is a liaison and provides cultural mediation between the health care system, social services, and the community. Acts as a trusted member, and has a close understanding of, the ethnicity, language, socio-economic status, and life experiences of the community served. Assists people to gain access to needed services and builds individual, community and system capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, patient navigation and follow-up, community health education and information, informal counseling, social support, advocacy, and participation in clinical research. In collaboration with program managers, provide daily oversight and evaluation of assigned staff members including managing payroll and annual evaluations. WHAT IS EXPECTED (ESSENTIAL JOB FUNCTIONS): Supervise daily activities of program to ensure efficient operations, including facilitating team meetings as directed by program manager Maintain outcomes data and dashboard to allow for program evaluation Interview, select, train, supervise, counsel, and recommend termination of assigned employees in partnership with program manager. Supervise assigned staff and provide coverage when staff is on PTO Perform administrative duties related to payroll and other assigned responsibilities Investigate, resolve and document resolution of patient or staff complaints Communicate with team members regarding protocols and practices related to their assigned area Provide insight as a subject matter expert on strategic initiatives related to the role of the Community Health Worker The information contained in this job description is intended to describe the essential job functions required of those assigned to this job. Diploma/GED Equivalent - EXPERIENCE - 3 Years of Experience - CERTIFICATION/LICENSE/REGISTRATION - Community Health Worker (CHW): Certified Community Health Worker from Texas Department of State Health Services Required. Cert Community Hlth Wkr Instr (CHW-I): Certified Community Health Worker Instructor required within 6 months of hire.
