JOB SUMMARYThe Field Engagement Specialist is responsible for engaging STAR+PLUS Members and families in service coordination by facilitating communication, assisting in health systems navigating to promote access, encouraging the member to adopt healthy behaviors, and empowering the member, caregiver and /or family to interact with health/social services. Additionally, the Field Engagement Specialist supports the Members assigned Case Manager through an integrated service coordination approach that involves engaging with the Members community support.
